About the Role

Join our Program of All-Inclusive Care for the Elderly (PACE) — a nationally recognized model that empowers seniors to live independently while receiving comprehensive, personalized healthcare. As a Primary Care Physician, you’ll be part of a collaborative Interdisciplinary Team (IDT) delivering whole-person care to older adults across home, clinic, hospital, and nursing home settings.

What You’ll Do

Provide ongoing, holistic primary care to PACE participants
Perform in-person assessments and develop individualised care plans
Collaborate with the IDT to manage transitions of care (admissions, discharges, referrals)
Oversee clinical care in nursing homes and hospitals
Supervise and support nurse practitioners and other care team members
Participate in IDT meetings and a shared on-call rotation (including nights)

What We’re Looking For

MD or DO from an accredited medical school
Completion of an ACGME-accredited residency program
Board Certified in Internal Medicine, Family Medicine, or Geriatrics (preferred)
Unrestricted California medical license and active DEA registration
Eligible to provide services to Medicare and Medicaid beneficiaries
