Overview:

This position supports the Program of All-Inclusive Care for the Elderly (PACE), which empowers seniors to maintain independence and remain in their homes and communities. The model emphasizes comprehensive, holistic care that addresses not just medical needs, but the whole person. Physicians work collaboratively within an Interdisciplinary Team (IDT) to evaluate participant health and develop individualized care plans.

Position Summary:

The Primary Care Physician conducts in-person assessments and coordinates care plans alongside the IDT. The role includes oversight of participant care across various settings such as nursing homes and hospitals, with responsibility for managing admissions, discharges, and referrals to specialists. The physician also provides clinical leadership and support to nurse practitioners and other care team members.

Key Responsibilities:

• Provide comprehensive primary care and ongoing medical management to participants
• Coordinate care transitions in collaboration with hospitals, nursing facilities, and the IDT
• Participate actively in IDT meetings to review cases, update care plans, and support 24-hour care coverage
• Conduct regular visits to nursing home residents based on individual care needs
• Participate in a rotating on-call schedule, including nights.

Qualifications:

• Medical degree (MD or DO) and completion of an ACGME-accredited residency program
• Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred)
• Valid and unrestricted medical license in the state of practice
• Active DEA registration and controlled substance certification
• Eligibility to provide services to Medicare and Medicaid beneficiaries
